At your first visit, you will be asked to fill out medical and dental history forms and will have the opportunity to review and sign our HIPAA forms and office policies digitally.  In order to best serve you, please plan to provide documentation specific to your medical, dental, and insurance information.  This includes items such as:  current address & phone number, dental insurance provider and contact information (if applicable), & prior dental records (if desired).
